However you would naturally say it. Some people write in the present tense as though it has arrived, others write a plain description, others write a prayer. The wording matters because you will reread it, so use language that does not feel borrowed.
Dates you can hold without flinching. Some practices treat the date as a commitment and others as an intention, and either works here. What does not work is a date you already know is fictional, since it teaches you to discount the whole board.
Daily is the usual answer and the practical reason is repetition rather than ritual. Setting the saved board as a phone wallpaper achieves it without any effort, which is why most people end up doing that rather than scheduling time to look.
